The Anemia Management Program Coordinator will facilitate and coordinate eligible patients under the hospital’s Anemia Management Program. The Anemia Management Program Coordinator maintains and oversees the hospital’s approved anemia management patient profile platform. The Anemia Management Program Coordinator performs clinical assessment of patients enrolled in the program using the platform and available patient data from the hospital’s electronic health record.
The Anemia Management Program Coordinator will perform clinical assessments of patients may include, but are not limited to, use and safety of anemia treatments pre-operatively including the coordination of administration of IV iron and erythropoietic stimulating agents. The Anemia Management Program Coordinator will review, coordination and evaluation of platform program documents in regard to other peri-operative risks and care planning, assessing the ischemic risks, bleeding and clotting risks, infectious risks, oxygenation and other risks prior to referral to infusion provider, during treatment or intervention, or post-operative follow-up.
The Anemia Management Program Coordinator will demonstrate excellence in customer service and public relations to promote a positive professional image of the Hospital. The Anemia Management Program Coordinator will work independently with attention to detail and possesses the necessary knowledge of all aspects of daily operations. The Anemia Management Coordinator establishes a process to track, audit and analyze key performance metrics and offers recommendations to address areas of concern to improve safety and treatment efficacy, and to reduce costs.
The ideal candidate for Anemia Management Program Coordinator should have 3 years of experience in leadership and performance/quality improvement roles in nursing and/or infusion service with 5 years recent experience in Nursing or Clinical Laboratory experience in Emergency Medicine, Critical Care or Infusion Service preferred. Experience in performance improvement data reporting and analysis. The Anemia Management Program Coordinator should have State or National license as a Registered Nurse (RN/BSN) or Clinical Lab Scientist/Medical Technologist- MT(ASCP) or Clinical Blood Bank Specialist- SBB(ASCP).
